Medical Assessments

For those who need in-home care, a medical assessment is critical to determining the scope of in-home care and how those services will be covered by your public or private insurance. Before you start any process, it's always a good idea to  first consult with a physician or medical provider.

Medicaid

Reach out to your local county office to arrange how you can get an assessment. 

Veteran Affairs

Reach out to your closest VA office to arrange how you can get an assessment. 

Private Insurance

Consult with your insurance company first. See below if you are in need of coverage.

Payments

We work closely with case managers to review each client’s assessment and determine the specific services that will best meet their needs. Based on this evaluation, we identify which services are authorized for coverage. Together, we establish an authorization agreement that outlines the approved services and the client’s covered budget.

Then our team carefully tracks all billable service hours to ensure accuracy and transparency. We handle the submission of service documentation to the appropriate organization of coverage, such as insurance providers. Once processed, we take full responsibility for ensuring timely payment to the agency.

In addition to billing and authorization, we manage compliance and reporting to maintain accountability. This includes verifying that all services were provided as authorized and that records are complete and accurate. Our detailed tracking and reporting help ensure quality care, financial integrity, and regulatory compliance.

CFSS Program

Community First Services and Supports (CFSS) is a Minnesota program designed to help people with disabilities, chronic conditions, or age-related needs live more independently in their homes and communities. It replaces the former Personal Care Assistance (PCA) and Consumer Support Grant (CSG) programs. CFSS offers flexible, person-centered services such as help with daily living activities (bathing, dressing, grooming), health-related tasks, household chores, and community participation.

To get started, individuals should contact their county or tribal human services office to request a MnCHOICES assessment to determine eligibility and level of need. After the assessment, participants receive information about CFSS and a list of consultation services providers.
